Part Time: Working 30 hours per week Monday to Friday

WHY THIS ROLE?

We are delighted to offer this internal opportunity for a Registered Nurse looking to further develop their skills within specialist palliative and end of life care. Working within our Community Team, you'll have the opportunity to broaden your clinical experience, develop specialist knowledge and play a key role in supporting patients and families across our community services.

As a Community Staff Nurse, you'll work alongside our experienced Community Nurse Specialists and wider multidisciplinary team, providing holistic, person-centred care to patients with life-limiting illnesses. You'll gain valuable experience in assessment, symptom management, advance care planning and community-based palliative care, whilst receiving support and guidance from experienced colleagues committed to your professional development.

WHAT WILL THE ROLE INVOLVE?

As a Community Staff Nurse, you will work as part of our specialist community palliative care team, supporting patients and families across our community services. Your responsibilities will include:

  • Undertaking assessments of patients with palliative and end of life care needs within their own homes, clinics and virtual settings
  • Supporting patients and families through holistic, person-centred care that considers physical, emotional, social and spiritual needs
  • Contributing to complex symptom management and the delivery of high-quality palliative care
  • Developing and implementing care plans in partnership with patients, families and healthcare professionals
  • Working closely with Community Nurse Specialists, GPs, health and social care professionals and the wider multidisciplinary team
  • Supporting bereavement assessments and referring patients and families to appropriate Dorothy House services
  • Participating in the Dorothy House Advice Line rota following completion of training and competencies
  • Supporting education, mentorship and learning opportunities for students, colleagues and volunteers
  • Maintaining accurate patient records and contributing to audit, quality improvement and service development activities

ABOUT YOU

Please Note that only existing Dorothy House employees / volunteers are eligible to apply for this role.

Essential requirements:

  • First Level Registered Nurse with a relevant post-basic qualification
  • Clinical experience at Band 5 level or equivalent
  • Excellent communication and interpersonal skills, particularly when supporting patients with complex care needs
  • Experience working collaboratively within a multidisciplinary team
  • Evidence of continued professional development
  • A compassionate and supportive approach to caring for patients and families during their end-of-life journey
  • Full UK driving licence with access to a vehicle

Desirable requirements:

  • Experience managing a caseload independently within a relevant clinical specialty
  • Knowledge of palliative and end of life care principles
  • ENB 998 or equivalent experience in teaching
  • Research or audit experience
  • Community nursing experience or a relevant community-based qualification
