Description
This is an exciting opportunity for an experienced palliative care Registered nurse to take up the position of Frailty nurse specialist for a 6 month project. The Nurse Specialist will work alongside the Banes Frailty Advanced Nurse Practitioner, the local authority and the BANES Wellbeing hub to explore a collaborative approach to improved support for people with frailty and end of life care needs. The post holder will be provided with mentorship support by CPCT Clinical lead and Admiral nurse.

FRAILTY NURSE SPECIALIST | Band 6 £35,392 - £42,618 per annum (pro-rata)
This is a fixed term secondment position for a duration of 6 months.
In this role your duties will include:
- To participate in the development of services in line with the organisational strategic direction, to assist individuals to support "Death is a part of life", "Living Well", to lead to a "Peaceful death", incorporating a "Supported bereavement" and participate in developing/maintaining an "increase in income".
- To work with the Frailty ANP through a project approach to contribute to continuous quality improvement and service development to improve patient, family, carer and staff experience of frailty.
- To work as part of a CPCT, to provide specialist palliative care to those with frailty and their families.
- To act as a resource to provide specialist clinical advice, support and education to health and social care colleagues, students and other professionals.
- To provide staff support on a day to day basis, to address staff issues as they arise and to promote methods, such as clinical supervision, for personal and professional development
- To initiate and deliver formal training programmes in collaboration with the DH Education and Research and Training & Development Teams.
- To support an environment of open and honest accident and incident reporting and promote lessons learnt.
- To work with the Clinical Lead to arrange induction and mentorship programmes for new members of staff and volunteers.
- To participate in research, as appropriate, and apply relevant findings to practice and teaching.

HOURS OF WORK
This is a part time. You will be working 15 hours per week across 2 days.
ABOUT YOU
Please Note that only existing Dorothy House employees / volunteers are eligible to apply for this role.
Essential requirements:
- First Level Nurse qualification with clinical experience at Band 6 level or equivalent
- Clinical experience in relevant speciality with evidence of palliative care knowledge
- Experience facilitating Learning and Practice
- Excellent communication/interpersonal skills
- Ability to work alongside and support other members of the team
- Ability to work independently and manage own caseload
- Ongoing evidence of professional development
- UK driving license with access to a vehicle*
Desirable requirements:
- Full Independent Nurse Prescriber (or be willing to undertake this course)
- Clinical assessment and analytical skills- PACR course (or be willing to undertake)
- Experience in teaching/education
- Research skills
- Experience working within the community
- Relevant degree or post graduate qualification(s)
