Description

We are seeking a dedicated Clinical Lead to our Community Palliative Care Team (CPCT) to provide exceptional care and support to our patients and their loved ones. As a Clinical Lead, you will play a pivotal role in our palliative care delivery, driving continuous quality improvement, and fostering strong partnerships within our community.

CLINCIAL LEAD - COMMUNITY PALLIATIVE CARE TEAM (CPCT) | Band 7 £43,742 - £50,056 per annum

12 month fixed term role

Based at our Winsley office as well as working in our community, you will participate in the development of services in line with the organisational strategic direction, to assist individuals to support "Death is a part of life", "Living Well", to lead to a "Peaceful death", incorporating a "Supported bereavement" and participate in developing/maintaining an "increase in income".

Your duties will include:

  • Provide coaching support to develop a new model of community palliative care, fostering collaborative relationships within Dorothy House services and external partner organizations to enhance end-of-life services.
  • Work within a Community Palliative Care Team (CPCT) to deliver specialized palliative care to patients and families, advocating for the importance of nursing within the multidisciplinary team.
  • Lead a culture of continuous quality improvement and service development within CPCTs, demonstrating organizational values and fostering strong partnerships with external organizations.
  • Manage a limited caseload, providing specialist palliative care in partnership with health and social care colleagues, ensuring competence and adherence to clinical standards.
  • Provide day-to-day coaching and support to CPCT leads, participating in recruitment, training, and development activities in line with Dorothy House HR policies.
  • Represent the area team at internal and external forums, contributing to the development and monitoring of clinical practice and service provision.
  • Provide leadership support for the development, maintenance, and monitoring of quality specialist palliative care, reporting risks, issues, and service developments to senior management.
  • Lead education initiatives to enhance learning opportunities for CPCT staff internally and externally, participate in research and clinical audit activities, and adhere to professional standards and regulations.
  • Utilize reflective skills to evaluate current practice, participate in clinical audit, and identify areas for research to inform evidence-based practice and teaching within the palliative care setting.
  • Adhere to professional codes of conduct, legal regulations, and organizational policies, participate in clinical supervision and professional development activities, and maintain accurate records to ensure high-quality patient care and professional accountability.

HOURS OF WORK

This is a fixed term, part time position. You will be working 34 hours per week (4 days at 8.5 hours per day)

ABOUT YOU

Please Note that only existing Dorothy House employees / volunteers are eligible to apply for this role.

Essential requirements:

  • Registered Nurse
  • Postgraduate or equivalent level of knowledge in a relevant subject
  • Teaching and Assessing qualification
  • Independent Prescriber or working towards qualification
  • Clinical experience in specialist palliative care
  • Management/ leadership skills experience
  • Clinical assessment and analytical skills
  • Self-motivated and able to work as part of a team including the multi-disciplinary Team
  • Ability to analyse data and research in practice
  • Excellent communication and interpersonal skills
  • Confident in use of IT systems including patient systems
  • Full UK driving licence with access to a vehicle*

*Essential to support the ability to deliver patient services across a geographical patch
